Is Adhearsion some kind of Ruby on Rails plugin?

No! Adhearsion is an entire metaprogramming framework in itself. It does integrate with Rails, but it should not be confused with a web development framework. Adhearsion’s purpose is very specific: improve VoIP development. This may be accomplished with the use of web integration, but Adhearsion is in no way limited to that.
